On admission, LifeChoice teams in Chicago and nearby communities thoroughly review each veteran patient's military history. This allows us to better understand their medical, emotional, and spiritual needs and enables us to honor their service through personalized recognition activities such as memorial services, pinning ceremonies, bedside salutes, and special tributes.

LifeChoice’s experienced teams understand the complex challenges some veterans face at the end of life, including financial concerns, navigating benefits, coping with post-traumatic stress disorder (PTSD), and addressing unresolved emotional struggles linked to military service.

The VA, community-based healthcare providers, or veterans themselves can refer patients to LifeChoice for hospice services. Veterans and their families throughout Chicago and the Chicagoland area are also welcome to contact us directly to learn how we can support them.

• Full bereavement support services for families, including grief programs, support groups, and memorial services for up to 13 months after a loved one’s passing
