Many families begin hospice care after a doctor or hospital recommends it — but you can also call us directly. Early referrals help your loved one get relief sooner: pain control, comfort care, emotional support, and time together without added stress. The earlier we start, the more support we can give.

After the referral, our clinical team gathers the necessary medical history and records. We coordinate with doctors, hospitals, and other care providers to confirm that hospice is the right fit — both medically and emotionally. It’s not just about paperwork; it’s about making sure we understand the full picture before we step in.

Before care begins, we’ll check your loved one’s insurance coverage and explain what’s included. Hospice is covered by Medicare, Medicaid, and most private insurance plans. If there are any costs not covered, we’ll explain them clearly — and help you figure out the best path forward.

Once eligibility and benefits are confirmed, we reach out to the family. This is often one of the most important conversations we have. We’ll explain how hospice works, what support is available, and how our team becomes part of yours. We’ll also listen — really listen — to your hopes, your fears, and your questions. We’ll take the time to walk through it all, together.

After our initial conversation, a LifeChoice nurse will schedule an in-home (or facility) visit. This first assessment helps us understand your loved one’s current condition, symptoms, and care needs. It’s also a chance to talk face-to-face, build trust, and make sure everyone feels comfortable and confident before services begin. Our goal isn’t just to provide care — it’s to help you feel ready.
