The Site may allow you to submit information including your name, telephone number, email address, insurance information, date of birth, referral information, feedback, or other information that you voluntarily provide.

Do not submit unnecessary medical information through general contact, consultation, or feedback forms. When a form specifically requests health, insurance, or referral information, provide only the information reasonably necessary for the stated purpose.

Submission of a form acknowledging our Privacy Policy does not constitute a HIPAA authorization for uses or disclosures that otherwise require a valid authorization under applicable law.
